- MD/DO – Family Medicine
- Provider must prescribe
- BE, BE, CSR & BLS
- Background: Family Medicine – PEDS, Adults & Geriatrics
- 60-90 day credentialing
- Outpatient (12-16 PPD)
- Staff: 1 MD, 2 APP & Support Staff
- EMR: EPIC – must be proficient – Dragon Dictation
