Job Description

BC or BE in IM/FM
Start: Immediately upon completion of credentialing
Dates: 6-12 months (ongoing)
Schedule: FT - Monday - Friday; Flexible to support one evening clinic rotation 10a - 7p and Saturday clinic 8a - 5p (Saturday rotation is 1 every 10 weeks currently)
No call
Outpatient (2.5 per hour)
Staff: 6 + Psych NP, Each provider is part of a “ team pod” of 3-4 providers; each with 1 primary MA/1 support MA, 1 RN and 1 CSRS (referrals/medical records); In addition, to clinic has two BHCs and Social Worker available.
EMR: Epic
BLS & ACLS
FM/IM Experience required
Collaborating physician available
Provider must prescribe
10-12 weeks credentialing
